The camera toolbar plugin let’s us add a button and show a message box.
A. How can I open a new window from the button? We want to show thumbnails for PTZ positions.
B. Can we add our own context menu with several sub-menus here?

The MIP SDK has no functionality to do menu or sub-items on the button on the toolbar. I am guessing it would be possible to let the button open a new window with additional choices.
